According to Georgia law, children must be five years old by September 1 in order to enroll in kindergarten. To enroll in first grade, children must be six years old by September 1. To enroll in a Fulton County school, the student's parent or legal guardian must be a resident of the school system.

Registration Requirements

Immunization certificate - The State of Georgia requires students to be immunized against measles, mumps, polio, rubella, whooping cough, tetanus, and diphtheria. The Varicella vaccine has also been added to the list of required vaccines for attendance in Georgia public schools. Students born on or after January 1, 1992, must have completed the hepatitis B series. All children entering 6th grade (middle school) are required to have the chicken pox vaccination or proof of immunity. Immunization information must be on the proper Georgia Department of Human Resources Form 3231. These are available through the Fulton County Health Department or local physicians.

Certificate of eye, ear, and dental screening - Use of Georgia Department of Human Resources certificate Form 3300 is required. The form must be signed by an authorized representative of the Health Department or a licensed physician.

Birth Certificate - Parents must submit a government issued original certified copy. Hospital certificates are not acceptable. A photocopy of the birth certificate will be made and the original will be returned to the parent/guardian at registration.

Proof of residency - Two (2) current signed and dated official documents such as utility bills that have the parent or guardian's name and address are required and a notarized Affidavit of Residence.

Social Security Card - A photocopy of the card will be made and the original will be returned to the parent/guardian at registration.

Academic records - If possible, bring your child's most recent report card, the withdrawal form from her/his previous school and/or transcripts to assist us in proper placement of your child.
